An NDIS Service Agreement – What is it?
A Service Agreement is a different document to your NDIS Plan. The NDIS Plan explains what you want to do, what your goals are, and the support you need to achieve your goals.
An NDIS Service Agreement
A Service Agreement outlines the working relationship you have with your service provider. It outlines how you and your service provider both agree on the supports you will receive and how these supports will be delivered. Many registered providers will have their own terms and expectations. In other cases, your Plan Manager and Support Coordinator will likely be able to provide you with a simple NDIS service agreement. A Service Agreement can be developed by yourself, or with a service provider.
In developing an agreement, these are the necessary considerations:
- Supports to be included
- Participant expectations (e.g. required level of service quality)
- Service provider expectations (e.g. cooperation)
- Payment terms
- Change terms and process for making changes
- Termination terms and process for ending a contract
- Timeframes
- Point of contact
- Signatures

- Service bookings should reflect the Agreement
- Attaching the NDIS Plan to the Agreement
- Keeping a copy of the Agreement for your own records
- Procedure for resolving disputes
